Load runs through the axle bore every time this tool moves across a surface, which is why that bore is the first place wear shows up. Once it opens up even slightly, the wheel sits loose in its mount and lets the housing tip or drag, which throws off how evenly the brush roll makes contact underneath.
A wheel worn flat on one side from turning the same direction repeatedly, or one that has stopped spinning freely by hand, is past the point where anything short of replacement helps.
